Usually there is a clear process documented within agreed laid-down procedures, with an emphasis on improving the efficiency of these procedures. Clinicians are encouraged to try and interpret the meaning of various symptoms reported and may well have, as part of the suicide risk assessment procedure, a questionnaire in printed or remembered form. As mentioned earlier, the UK Government White Paper, Saving Lives — Our Healthier Nation (HM Government, 1999), listed one of the recommended steps towards further suicide rate reduction as “continued professional training about assessment of suicide risk”.

There is, however, a distinction between ‘completed suicide’ and ‘attempted suicide’. For the purpose of this book, I am defining attempted suicide as a serious attempt to end one’s life which has been unsuccessful for whatever reason. I put much emphasis on the word serious to distinguish it from other self-harming behaviours which I will discuss below. I mentioned earlier, in one of the definitions, the idea of pain playing an important part in suicidal thinking and behaviour. Schneidman (1993) was very interested in this aspect, describing unbearable mental pain as “psychache”.

Recent pay increases and the rising costs of drugs have also stretched resources vastly. Many mental healthcare trusts have been encouraged to work only with ‘serious mental illness’ (SMI), turning away others whose degree of distress symptoms have been thought to be treatable within primary care.
